About this Care Path
For patients with stronger non-emergency illness symptoms who may need broader supportive care during a mobile medical visit.
How the visit works
- 1. You request care and share what is going on.
- 2. A clinician reviews your request before anything is provided.
- 3. Your care team confirms the plan and the cost with you.
- 4. Care is provided at your location by a licensed clinician.
Clinical model
A Baseline Medical Registered Nurse performs the on-site assessment and care execution. A Baseline Medical Nurse Practitioner guides appropriateness, protocol decisions, symptom management, and escalation.
Choosing a Care Path tells the clinical team what kind of support you are looking for. It does not select your treatment: that is decided on assessment, and the cost is confirmed with you before anything is provided.
Safety and escalation
Treatment remains provider-guided and depends on symptoms, vitals, and eligibility.
If you have severe or rapidly worsening symptoms, chest pain, trouble breathing, fainting, confusion, or you believe this is an emergency, call 911 or go to the nearest emergency department.
Care Paths are designed to help patients choose the type of support they are looking for. Final treatment options are selected by the clinical team based on assessment, safety, availability, and patient needs. Some medications, vitamins, or fluids may not be appropriate for every patient. Baseline Medical does not provide emergency care. For chest pain, trouble breathing, stroke-like symptoms, severe allergic reaction, severe abdominal pain, fainting, confusion, uncontrolled bleeding, or any emergency symptoms, call 911 or seek emergency care immediately.
