What is Baseline Medical Ship-to-Home?
Ship-to-Home is a Baseline Medical service that is coming soon. A licensed Baseline clinician reviews eligibility in a virtual visit, and approved items ship to your home after the clinical and membership requirements are met.
How does Ship-to-Home work?
You choose an option and request a virtual eligibility visit. A Baseline clinician licensed where you are located makes an independent clinical decision. If the option is appropriate, an active qualifying membership is required, fulfillment requirements are confirmed, and approved items ship to you.
Which membership do I need for Ship-to-Home?
Access ($39/month) is the entry membership for Ship-to-Home. It includes Ship-to-Home access, member-only ordering, and member pricing on eligible services. Premier ($239/month) and Executive ($499/month) also include Ship-to-Home access, along with their other Baseline membership benefits. Access is coming soon.
Is Ship-to-Home available now?
Not yet. Ship-to-Home is coming soon and cannot be requested today.
Will I need a virtual appointment?
Yes. Every Ship-to-Home option requires a virtual eligibility visit with a licensed Baseline clinician before anything can be approved.
Who reviews eligibility?
A Baseline clinician who is licensed where you are physically located during the visit reviews your eligibility and decides what is clinically appropriate.
Does my location matter for a Ship-to-Home visit?
Yes. For the virtual visit, what matters is where you are physically located during the visit, because your clinician must be licensed there. That is separate from the Mobile Medical service area, and separate from where approved items are shipped.
Does membership determine eligibility?
No. Membership never determines clinical eligibility, clinical judgment, or approval. Clinical review always comes first.
Does membership include the product?
No. Membership does not include Ship-to-Home items, pharmacy charges, shipping, or supplies. Those costs are separate.
Is NAD+ a Ship-to-Home option?
Yes. NAD+ is a Ship-to-Home option, provided only when a licensed Baseline clinician decides it is appropriate for you.

Ship-to-Home begins with a virtual eligibility visit and ships approved items to you. Mobile Medical brings a Registered Nurse to your location, with a Nurse Practitioner joining virtually, for in-person care.
Does membership change where Mobile Medical is available?
No. Mobile Medical is in-person care delivered to your location, so that location must be inside Baseline’s current service area. Having Access, Premier, Executive, or any other Baseline membership does not make a location outside the service area eligible. Service availability is determined by Baseline’s service-area and operational rules.
